Appointment Booking Regulations
Glossary of terms
1. The Clinic – Mokotooth Prestige Dental based at Chocimska 20/102 street, 00-791 Warszawa.
2. Appointment - Dental appointment and procedure offered by the Clinic.
3. Patient - The Person making a booking of the appointment
4. Booking Fee - the fee, payed to the Clinic by the Patient to book an appointment
Making appointments for patients
1. The prior payment by the Patient of the Booking Fee is required for making an appointment reservation.
2. After the Booking Fee has been recorded by the Clinic, a Clinic’s employee will contact the Patient by phone or e-mail to book the Appointment.
3. Until the Booking Fee is recorded, the visit date is temporarily booked. If the Booking Fee is not paid within the specified time, it will be automatically canceled
Booking Fee
1. The Booking Fee is 300 zł.
2. The Booking Fee is not a payment for the dental procedure served during the appointment. The costs of the procedure(s) are covered by the Patient on site after the service is provided.
3. The price of the appointment to be paid by the Patient is reduced by the amount of the Booking Fee.
4. The Booking Fee is not refundable.
Payments
1. The Booking Fee can be paid using the following methods:
- Bank transfer to the Clinic's bank account:

- In cash or by payment/credit card at the Clinic
2. The patient pays for the appointment immediately after its completion in cash or by card.
Moving and canceling appointments
1. Appointments can be rescheduled no later than 72 hours before the scheduled visit date.
2. If the Patient informs the Clinic about the change of the date of the scheduled appointment later than 72 hours before the previously booked date, the payment of new Booking Fee is required.
Personal data protection
1. In accordance with the Act of August 29, 1997 on the protection of personal data (and subsequent amendments), the Clinic will ensure the protection of all personal data obtained during the booking process.
2. Patients' personal data are processed solely for the purpose of booking an Appointment.
3. In the process of booking an appointment, the Clinic collects the following patient data:
- - Name, Surname – in order to identify the Booking Fee and book an appointment for the Patient
- - Email Address – in order to confirm receiving of the Booking Fee and to contact the Patient regarding preparation for the Appointment
- - Phone number – in order to arrange an appointment by phone
4. The patient has the right to access his/her personal data at any time and the right to correct it and request its deletion. This right is exercised by sending a signed application to the Clinic's address.
Regulations for paying a deposit towards the implementation of the Dental Treatment
Glossary of terms
For the purposes of this document, individual phrases have the following wording:
1. The Clinic – Mokotooth Prestige Dental based at Chocimska 20/102 street, 00-791 Warszawa.
2. The Treatment - the dental treatment offered and provided by the Clinic
3. The Patient - the person, which order the Treatment
4. Down payment - down payment within the meaning of Art. 394 of the Act of April 23, 1964 towards expenses related to the performance of Treatments by Mokotooth Prestige Dental. The amount of the down payment takes into account the amount of expenses necessary to incur in connection with the providing of the Treatment and booking the date of the Treatment.
Amount and date of payment of the deposit (down payment)
1. The amount of the down payment depends each time on the type of Treatment and results directly from the amount of expenses incurred by the Clinic in connection with the providing of the Treatment, i.e. in particular costs related to booking the availability of the doctor and staff providing the Treatment for its duration, costs of funds and materials used during the Treatment.
2. The Patient will pay the down payment no later than 14 days before the date of the scheduled Treatment.
3. The down payment can be paid using the following methods:
- Bank transfer to the Clinic's bank account:

- In cash or by payment/credit card at the Clinic
4. The Treatment date is booked after the down payment is credited to the Clinic's bank account, provided that the deposit is paid on time..
5. Failure to pay the down payment within the agreed deadline is tantamount to the Patient's resignation from the Treatment. The Clinic is not obliged to additionally inform the Patient about the cancellation of the Treatment.
6. The difference between the Treatment cost and the paid down payment is payable by the Patient on the day of the Treatment, after its completion, at the Clinic’s reception desk in cash or by card.
Moving and canceling the appointments
1. If the Treatment is not provided due to the Patient's fault, e.g. if the Patient cancels the Treatment date or the Patient fails to appear at the Clinic on the booked date or if the Patient is late by more than 10 minutes making it impossible to perform the Treatment, the deposit paid will be forfeited to the Clinic.
2. If the Treatment is not provided due to the fault of the Clinic, i.e. if the Treatment is canceled by the Clinic, the deposit paid will be returned to the Patient.
